Payment and Insurance

Reynoldsburg Family Dental Financial Policy

The doctors and staff of Reynoldsburg Family Dental thank you for your trust in us. We have several options to make your dentistry affordable. We would like to take the opportunity to inform you of these options in order to avoid any misunderstanding about how we collect our fees for services rendered.

envelopeDental Insurance:

The fees we charge for services rendered to those who are insured are our customary fees charged to all patients for similar services. Each patient is financially responsible for his or her own account. We are happy to cooperate with any patient whose treatment is covered by dental insurance. We only ask that you are fully aware of any limitations of the benefits provided. Your insurance policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage offered. As a service to you, we will complete all forms pertaining to your claim and send them to your insurance company. If your dental insurance company does not pay our office within 60 days we request that the balance be paid in full. If you have dual insurance, we will accept assignment of benefits from the primary plan only. Any balance after insurance pays remains the patient's responsibility.

Should you need any treatment, we will provide you with a treatment plan that will be itemized. It will show the services needed, an estimation of insurance coverage and the patient's estimated portion. In an effort to keep dental costs down, while maintaining a high level of professional care, we have established the following payment policy for the use of our patients.

Fees may be paid as follows:

  1. Payment is due at the time of service. We will accept cash, check, Visa, MasterCard, Discover and debit cards. A 5% courtesy is offered to those patients who pay in full with cash at the time of service.
  2. For patients with insurance, the estimated portion is due at the time of service. Payment of balance is due in full, according to financial arrangements, if your insurance does not pay the estimated percentage within 60 days. You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates. Patients may choose to pay one-half of the fee prior to treatment and the remaining half prior to the final treatment appointment.
  3. We also have a dental financing plan available to our patients called CareCredit. CareCredit allows patients who qualify to make payments over time with no down payment. Patients who have used this plan so far have given us very positive feedback at the flexibility it gives them to proceed with needed dental care. You can apply in our office or apply online. If you have questions about CareCredit, please ask our staff.

Minors: If you are the parent of a minor that is seen in our practice, we request that payment be made at the time of service.
